Have you tried the LaTeX typesetting system? It's used mainly for scientific writing but has gained some acceptance in writing general prose. Horribly unwieldy and tricky to learn, and probably useless to most people other than scientists and hobby typesetters, but the sheer freedom it gives you over the appearance of the document means you can spend weeks at a time fiddling about with settings and not get anything down other than the chapter titles.

 

But, by Christ, those titles will look fantastic.

 

Here's a recreation of a 16th century French bible typeset using LaTeX (not mine, I hasten to add).
