Actually Politics 101 would tell you that in many, maybe most, countries there are loads of unelected members in the cabinet.  Indeed in many you can't be an elected parliamentarian and be in the cabinet.  Wiki has a useful overview:

Even in the UK, the Prime Minister can effectively appoint anyone they want to Cabinet and validate it by making them a member of the House of Lords.  So they certainly don't have to be elected.  In fact in the UK there was a rule (up to 1926) that any MP who was made a Cabinet Minister had to submit himself to a parliamentary by-election to make sure that his constituents were happy with him doing so, 

This is a classic example of where people fervently demand that what we do in this particular country at this particular moment of time is the only way in which things can possibly be done and any alternative is is deeply evil and undemocratic.  Right up until it becomes convenient to change things and then the 'new normal' is championed with equal fervour and a complete forgetting of the immediate past.  So everyone says that the Chief Minister has to be an MHK - until Corkill has to go and Gelling, an MLC, takes over.  Or Bell claims that Minister must be MHKs - until it suits him to appoint Crookall.

What is actually important in a democracy is not who the Cabinet are, but the control that the elected legislature has over them, to hold them to account.  So the rule in the Isle of Man that Ministers have to be MHKs actually works against democracy because it means that a substantial proportion of the House of Keys has to back whatever nonsense the Cabinet puts forward.  We really need a different system or more elected members to balance that.

Do any of those with unelected ministers have a system where the person choosing the ministers isn't directly elected either as president or as head of a political party?

You only have to look at the top of the tree at DOI,  NB who was Director of Transport and he who dodged the Bus drivers crisis and was given promotion right in the middle of the trouble, he should have been told sort out this mess then can be promoted, then on the back of that we have IL who thought he won the lottery when he was made Director of Transport, BH retired and then RP was appointed Director of Highways that pissed of the Director of Works JR who soon got his little empire built with RP leaving after a short time ,  no wonder IT had it on his toes and got the fuck out, what Minister in the right mind would want to manage that bunch

Not usually initially, but political parties can change their leaders without recourse to the public and it can even happen in directly-elected systems (eg with Gerald Ford).  But of course the  real problems in the Manx system are that MLCs aren't elected and the number of politicians in the legislature in total is so small that the cabinet makes up such a high proportion of them.  That's even before you come to the scam of Departmental Members.

General knowledge to anyone who cares about how taxpayers money is wasted, the Manx CIA is probably run by the CS so have little to worry about.

But remember this is the DoI.  They'll probably break down the front doors of six people who have nothing to do with it, stop for a prolonged tea break and then forget all about it.

Have they still got six men at the DOI? I know they have loads of managers, but  have they got anyone to break down the door?

If you want to know what goes on at the DOI speak to the men on the ground and they will be happy to spill the beans on any shenanigans as they are treated worse that the public/taxpayers.

No problem these days. For years a 'friendly' person working at Manx Telecom could pass on any internet information required (emails, internet history etc). 
These days Manx Telecom, with dumping their shareholders, is a private company that 'treasury' has a significant share of. Getting info is relatively straightforward, IP tracking is a cinch.
